Male infertility is becoming more and more common nowadays but there is no need to lose your hopes to have a baby with your partner. Most cases of male infertility are treatable.
If the sperm count is lower than 15 million per ml then the underlying reason is diagnosed to ensure a proper treatment. Most of the times a varicocele (enlarged veins in Scrotum) or an obstructed vas deferens (a tube that carries sperms) are the reasons. They can be repaired surgically. Any underlying infection can also be the reason and the medication can do wonders to your sperm count. Hormonal therapies are also a good option if testosterone count comes out low.
Let’s explore some of the most common reasons for male infertility and their treatment:
1. Sperm Retrieval for Azoospermia (absence of Sperms in semen)
If the sperm count is low or zero in the patient’s semen, then they are retrieved with the help of minimally invasive surgical techniques:
- Sperm retrieval techniques are applied for artificial insemination (implanting your sperms into your partner’s womb to facilitate pregnancy).
- Gathering sperms from the bladder or any other areas which can be used for the intrauterine insemination (they are planted in your partner’s womb) to facilitate the fertilization process for a normal pregnancy.
- In these sperm collection procedures, a needle is inserted in the area to retrieve the sperms. Local anesthesia is given to the patient to make the procedure pain free.
- TESE (Testicular Sperm Extraction) is the most advanced technique utilized to retrieve sperms in case of azoospermia. In this procedure, general anesthesia is given to the patient. Then a very small portion of testicular tissue is removed and sperms are harvested from it. These sperms can then be implanted to your partner’s uterus. This will give rise to a very likely chance of conceiving a baby. This is a breakthrough in the field of male infertility treatment which has reduced the need for donor sperms immensely.
In this procedure, the patient is given general anesthesia. An electric probe is inserted in the rectum area and it produces some sine waves for a few seconds. These sine waves stimulate the nerves, which in turn contract the pelvic muscles, this ultimately leads to ejaculation. The semen is collected in a cup containing 3cc of the human tubal fluid buffer. This treatment has given hope to the patients with spinal cord injury or other paralysis patients. Now with the help of electroejaculation, they can enjoy fatherhood as well!
3. Sperm Ejaculation in Opposite direction (Retrograde Ejaculation) Treatment
This condition is another common reason for low or no sperm count in the semen. The sperms get ejaculated in the opposite direction of the urethra, that means inside the body (in the bladder) instead of outside. 4. Vasectomy Reversal
If you have gone through a sterilization procedure, then it can also be reversed making you fertile again. The procedure is quite successful in regaining male fertility. The Vas deferens is connected again during the surgery so that sperms can flow again and can get their way out from urethra.
5. Varicocele Treatment (Varicocelectomy)
A varicocele is a very common cause of the lack of sperm count in the semen. The tube that carries sperm becomes enlarged in structure. It may not cause any symptoms other than infertility. As the temperature of the sperm pouch (scrotum) is lower than the body temperature, this helps in keeping the sperm count high and keep them healthy. These tortuous and enlarged veins hamper this by providing a backward flow of blood from the body to the scrotum.
This varicocele can be treated surgically by tidying or removing the tortuous veins. Only the healthy veins are allowed to flow the blood. This boosts the sperm count many folds and thus treating infertility.
